I have gone through the ultrasound report you have mentioned.
This report is suggestive of infection in genitourinary organs.
So you should consult gynecologist and get done clinical examination and start empirical broad spectrum antibiotic for treatment.
If antibiotics work, you will improve in 1-2 weeks.
But if your symptoms are still persistent, you need laproscopic evaluation of adnexa. This will also help in identifying organism causing infection.
