Semen analysis report indicates semen infection.
You need few more investigations like routine hemogram,RBS,urine RE/ME,semen culture and sensitivity.
Proper treatment depend upon culture and sensitivity report.
Other findings like sperm count,sperm motility and morphology are within normal limit.
