it shows signs of ischemic stroke(cva) as mentioned there are lacunar infarcts. granulomas and calcification may be suggestive of old tuberculosis most likely. aspirin and statins should be given to patient if the diagnosis of ischemic stroke is confirmed. no need to intervene for calcified areas and granulomas. if active tuberculosis is suspected then treatment should be started after confirmation
