What Does Severe Pain Behind The Shoulder And Head Indicate?

I have occasional sharp throbbing pain sometimes near the top of my head, sometimes behind my shoulder blades or on my ribcage right side. It is never deep or dull as an organ pain. Always sharp for a second then nothing for ten seconds or so. Two or three ibuprofen help, heat often does as well.
