What Does " Diffuse Annular Hard Disc Bulge.." Mean?

Hi, i m detected diffuse annular hard disc bulge with broad based posterocentral soft disc protrusion and concentric chronic annular tear with focal flaval thickenining at L4-5, producing mild indentattion upon ventral thecal sac, mild narrowing of both lateral recess without compression of exiting nerve root. Mild ligamnntum flavum thickening with subtle annular hard disc bulge at l3-4 and minimal facetal hypertrophy and ligamentum flavum thickening at l2-3.
please explain me what all this means, since i have been suffering from lower back pain since 2011.

According to MRI report you have degenerative disc disease of lumbar spine.
Degenerative changes occurs in spine due to aging and development of weakness in annulus fibrosis which allows herniation of disc.

1.At L4-5 level,there is diffuse annular hard disc bulge with broad based posterocentral disc herniation and concentric chronic annular tear with focal ligamentum flavum thickenining.Herniated disc is causing mild indentation upon ventral thecal sac and mild narrowing of both lateral neural foramen without compression of exiting nerve root.
2.At L3-4 level there is mild ligamentum flavum thickening with subtle annular hard disc bulge.
3.At L2-3 level there is minimal facet joint hypertrophy and ligamentum flavum thickening .
Treatment for degenerative lumbar disease includes physiotherapy or surgery.
Effectiveness of both modalities are same.So,physiotherpay should be tried first. It causes strengthening of Para spinal muscles and it prevents further changes.
Surgery should be the last option.
There are also medicines like capsule pregabalin available for pain relief.
Treatment can be planned after consultation with neurosurgeon.
