General & Family Physician 's  Response
Hi,
Thanks for posting in HCM.
I understand your concern.
Based on the description and ultrasound report you have furnished, the following things can be inferred.
1. Menstrual cycle of 28 to 40 days is quite prolonged. It would be better that it needs to be regularized to normal of about 28 to 30 days by means of medications.
2. You are having an underdeveloped follicle on Day 14. Ideally, in a cycle of 28 days, ovulation takes place by 13 days. Since your cycle is prolonged, the follicle is not mature by 14 days and hence it would be difficult to predict when ovulation would take place.
With this situation, you would require medications to regularize your cycles and then undertake follicular studies. Based on the maturation status of the follicle, it can be decided weather or not ovulation inducing medications needs to be administered.
This would help your chances of conceiving.
Kindly consult Gynecologist or Infertility specialist for proper evaluation and management.
Hope the information provided would be helpful.
All the best.
