What Does My MRI Report Indicate?

my age is 65. My MRI report shows the L2-L3 disc show posterior protrusion impinging thecal sac. Abnormal marrow changes seen at L5-S1 with disctis. There is irregularity along the inferior endplate of L5 with minimal associated perivertebral soft tissue contiguous to both psoas. Features are suggestive of spondylo-discitis which may be inflammatory/ infective and koch's needs to be ruled out. A fracture? pathological seen at L5 on left side. Both S.I Joint reveal para articular sclerosis more on right side. Is surgery necessary.

Your report states that - there is an infective / inflammatory process which is causing bone marrow (inside the bone) changes at the level of L5-S1, Disc at the level of L5-S1 also shows infective changes including the end plate changes. Also seen involving the adjacent psoas muscle - These features are seen in the tuberculoses of the spine ( so it need to be ruled out first ).

A fracture seen in the L5 vertebrae - probably a pathological fracture ( if trauma is not there ).

At the level of L2-3 disc seen slipped out posteriorly causing pressure effect on the spinal chord - This condition is seen in case of age related degenerative changes, disc disease ( due to loss of hydration).

Sclerosis is seen in case reduced joint space with inflammatory changes - no initially treated conservatively, most of the cases it resolves - only in final stage the operative procedure is planned ( THR - total hip replacement).
