Hi
In my opinion, your fasting and postprandial sugar levels are raised, you have to go for a glucose tolerance test to confirm for diabetes mellitus. Discuss with your consultant the medications required to control your sugar levels.
Triglycerides are raised, you will have to start with medications to lower this, also make lifestyle modifications. You have not mentioned about the other parameters in lipid profile, if deranged that needs to be controlled too.
ESR is raised, indicates ongoing inflammatory process.
Coming to hemoglobin, guess you have put wrong results as it is always given in single or two digits and not in thousands if the units are in gm/DL. So regret not to opine on that.
Regards
