Welcome to health care magic.

1.There is a well defined anechoic cystic structure - which according to the report appears like features of pregnancy.
2.But pregnancy out side uterus called ectopic pregnancy - with yolk sac as mentioned.
3.There is a trace of the free fluid with in the pelvic region suggest - infective aetiology.
4.The location of the lesion is in the right adnexal region - which also possible of an ovarian lesion with complex features.
5.In this case you may need to get a BHCG levels evaluated and further cross sectional studies.

Good luck.
