What Do Persistent Headaches Indicate?

I am a healthy 48 Caucasian woman. Pmh hysterectomy as 38 for endometriosis, hypothyroidism, hypertryglycerides. My labs are good on meds. Pcp visit 9 mos ago did not show signs of menopause when hormones as checked. I have been having tension type headaches on a 24 hours a day, daily basis for a month now along with hot flashes several times a day. Never had as like this before. If my usual labs are good what is on your differential?
