Diarrhea and gurgling in stomach indicates intestinal infection. This itself can enhance back pain as a referred pain or may be a separate problem. This can also give the feeling of need to have motion.
Hence I would advise you the following:
-Send the sample of stools for a few days.
-Tests for blood.
-Ultrasonography of abdomen.
-X-ray of abdomen.
-Clinical evaluation, examination by your Doctor.
All these things will lead to a proper diagnosis and thus gives a plan of management. A course of an antibiotic active against intestinal infection can help you the best.
Take probiotics, plenty of oral fluids and soft diet. Any symptomatic and supportive management as needed or as advised by your Doctor.
