What Could Chest Pain With Difficulty In Breathing Indicate?

My fiance has pain in the left lower chest along his rib cage. It is tender to touch and there is no lumps, but there is what feels like a hard strand (almost reminding me of an adhesion). There is no redness or bruising. No injury or muscle strain. He is complaining of trouble breathing from the pain.
