Hi,

I have gone through your query.
Your friend is hypertensive and have high cholesterol level. Reason of missed beat could a Coronary Artery Disease (CAD). There may be blockage in arteries supplying blood to heart muscle.
I would suggest you to go for an Echocardiography and Coronary angiography as soon as possible after consulting a local cardiologist.
