Hi I have had a painful red rash that initially started out as only hives at night but then progressed to painful itchiness and constant redness with an almost scabby scaly look to it. It is now getting increasingly painful and it is starting to spread. What could it be?

After reading the query, in my opinion it appears to be Urticarial Vasculitis which is due to inflammation of small vessels which causes sudden redness with pain sensation rather than itching.
I suggest you to apply medium potency steroid cream application because are best effective in these condition. I would have opted for either mometasone or Fluticasone or cortisone cream. Do not pinch, pop or scratch or even touch the area unnecessarily as it can cause aggravation of the rash.
Wear cotton clothes and cotton inner garments which will give a soothing feeling to the skin. Stay away from extreme of cold environment.
