What Could A Bad Rash On Wrists And Arms Indicate?

My 5 year old son has a bad rash on his wrists and he's up both his arms. Firstly the doctor said it was his eczema playing up but after steroid cream making no difference they then said herpes. It is getting worse so i took him back today and they said scabies. No one else in the house is itchy or got any rashes?
