What Causes Unusual Angle Of Penis While Urinating?

The top section of my 3 year old sons penis always points at an unusual angle when he urinates. The angle can be very extreme at times and has caused his urine to spray his bum when he urinates. (still sits down when using the toilet). Average height and weight.
