For the past few days I have had a tingling sensation at the base of my penis (both underneath and on top) and testicles. when I urinate it can be slow to start, but feels almost orgasmic when I do start (does not burn or have pain). occasionally when I am simply sitting down it feels like I am getting aroused and may have an orgasm, but never do. when that happens there is usually semen that comes out first when I go to urinate. the symptoms subside for a few hours after sex. This is not the first time I have these symptoms and it is not clear what brings them on and what makes them go away. I do not have any STD s and am 38 years old.

This can be because of other symptoms too like, cystitis, urinary bladder infection or even chronic prostitis.
You need to do a urine examination and culture test for any particular pathogen. Also get sensitivity test done for the specific antibiotics.
