What Causes Testicular Pain?

Yesterday I was pushing a wheelbarrow with a flat tire that was full of dirt and I had to strain a bit and felt a pain in my left testicle. It hurt on and off yesterday and today when I woke up it was fine, but now in the middle of the day it is coming and going again.

Thanks for your query.
While you pushed the wheelbarrow with a flat Tyre, the muscles of the abdomen went into spasm which in turn pulled the cremaster muscle along with pull on the testicle. During this movement the testis can get a torsion compromising the blood supply . This gives you repeated pain.
I would advise you the following:
Get an examination by an Urologist / General Surgeon
Ultrasonography and color doppler
Scrotal support
Bed rest
Single stitch fixation of the testis if there is evidence of torsion.
