1.The history and symptoms suggest there was a trauma with
hematoma formation, other possible cause could be inflammatory cause post trauma.
2.The investigation of choice in this case will be
ultrasound examination of the
scrotum.
3.Ultrasound examination will help in evaluation of the cause of the pain, presence of inflammatory changes ( altered echo-texture with increased vascularity), presence of any hematoma.
4.Presence of free fluid or not, chord status, epididymus and vascular status.
5.The treatment is a course of antibiotics with symptomatic treatment, this can be done even before the scan, but evaluation of the cause will give better orientation of the treatment. Good luck.
