What Causes Swelling And Dark Discoloration In The Left Foot Of An Infant?

My nephew is 23 months old was his left foot is swollen and has turned purple one toe has burst open and bleeding. Drs say there is nothing wrong with him that they can find. But, there has to be something going on for it to swell like it has. Do you have any suggestions?
