What Causes Strings Of Blood In Semen And Urine?

What causes small strings of blood to come from penis. It is not accompanied with pain. I've seen it in both my seimen and urine. The 1st time, I saw it floating in my condom, mixed with my seimen. I've peed 4 times since and 3 of the 4 times I saw a small string of blood. What causes this?
