General Surgeon 's  Response
Hi.
With the history of urinary ladder thickening and enlarged prostate, the cause of the testicular pain and pain in the above site can be due to A condition of inflammation of the cord, epididymis and testicle called Epididymo-orchitis.
I would advise you the following:
Get the blood and urine tested including culture and sensitivity.
Ultrasonography and color doppler of the scrotum.
Get started on appropriate antibiotics and anti-inflammatory medicines.
Scrotal support.
Plenty of oral fluids.
