What Causes Shaky Hands, Painful Urination And Back Pain?

My hands have been shaking for the past couple of weeks. I have noticed that my legs and arms have been weak. Also had change in bowel habits that started about 6 months ago as well as lower back pain, painful urination, freezing cold testicles, fatigue, thick semen, headaches as well as fatigue.
