What Causes Severe Pain In The Urethra?

Hey Doc, 41 year old male. I have stinging type pain inside ureathra off and on for over a year (can move around). I have no issues urinating. The urologist I saw did a renal/prostate ultra sound to check before and after bladder retention. All was fine. My extensive blood work (including urine) came back perfect. No infections. Have had this issue off an on for a year. It waxes and wanes (can be gone for a month and then come back). Been with same woman for 20 years. Triggers seem to include masterbation/sex (but only sometimes). There is sometimes pain right at ejaculation (upper end of penile shaft). Could be me but seems worse when I am sitting. Sometimes, when I get up from a seated position, I can feel a pulling sensation near the penis. I am wondering if I have a pinched nerve issue as when this flares up I sometimes get sciatica that shoots down my leg into my foot. Been seeing a neuro for back/hip issues and 8 week PT that didn t do much. I know it s not killing me but it is depressing and annoying. Is an MRI of the back/pelvic warranted here? Thanks, -S

Hi,

These symptoms and signs are suggestive of STD especially if there is associated pain, fever, urethral discharge and recent sexual intercourse. So this is the first thing to rule out by doing STD testing. Second, it may be a sign of uroinfection or urinary stones and some other genital infection but this is less common.

In every case, antibiotic therapy may be required so test should be done as soon as possible so that appropriate treatment can be started. Till that try to drink plenty of fluids, avoid sexual intercourse and rest.
