What Causes Severe Pain In Lower Back And Stomach With Heavy Periods?

I have been having lower back And abdominal pain for about a week now. My periods are very irregular and very extremely heavy. I went to the emergency room last night and they done a transvaginal ultrasound and also checked my cervix. They said everything looked good to them no cysts. They gave me a copy Of my ultrasound and told Me to followup with my gyno. My question is what is Causing all of this pain?
