What Causes Severe Joint Pain And Fatigue In An Elderly Person?

I am a liver transplant recipient, 13 years out. I am a 68 yr old female. I had primary liver cancer. I have always done well, however lately I am having pretty severe joint and muscle pain as well as fatigue. I have my blood work done monthly and everything is stable. I am wondering if I should be concerned and insist on a more thorough check up?
