What Causes Several Coughing Episodes, Atrial Fibrillation And Reflux?

Cause for several coughing episode in a 94 y/o woman with aortic stenos, afib, gas/reflux issues. Cough is producing some phlegm. She reports dryness at the back of the throat and then has this strong cough that almost makes her vomit. She does have has some sinus symptoms. Lungs seem clear when I listen with stethoscope

As per my clinical experience, various possible issues pose with the current symptoms as:
@ The severe cough with vomit sensation may be in concern with viral infection, where lungs seems clear with stethoscope
@ The atrial fibrillation and sinus problems are secondary to the changes of stenotic valves which provide back pressure changes over the cardiac chambers, conduction defects at SA or AV node or Purkinjee his fibers

As there is no option for risky cardiac surgery at age of 94, now it is best recommended for conservative therapy with:
- Control of possible viral infection
- Proton pump inhibitor with antacid for reflux control
- Anti arrhythmic drugs as per the choice of her cardiologist
- Supportive care to control blood pressure, sugar levels
- Take care that she is not exposed to secondary smoke as well
