What Causes Redness In The Eyes While Suffering From Diabetes?

My friend is a diabetic,over weight,hasn t seen a doctor or taken care of his diabetes in 2yrs and his eyes are always bloodshot and his feet and ankles.are black and feel like leather. He s afraid to go the the hospital because of possible amputation. Is that likely? He doesn t feel good and it hurts to walk.
