What Causes Recurrent Pain In The Left Maxillary And Sphenoid Sinuses?

For the fourth time in about 18 months I have terrible pain in my left maxillary & sphenoid sinuses. The congestion is always localized to this area. The pain extends down to my teeth and back to my ear. I have intermittent fevers and continuous headaches. Terrible smell which causes bad breath. Was treated one time for a tooth infection rather than for my sinuses only to have to return to the doctor for an antibiotic targeting the sinuses. This time I have started using a Netti Pot. Last night about half an hour after I used it a chickpea size off-white chunk dropped down into my mouth. This was followed by half a dozen grey and black sunflower seed meat sized particles came out my nose. What in the world am I dealing with?
