What Causes Recurrent UTI S Along With Pain In The Rectum?

I am a 41 year old gay male. I get UTI s periodically. Symptoms include a constant burning sensation in my urethra, no discharge and sometimes pain around my rectum prior to a bowel movement or passing gas. I go to my GP and they do urine tests and STD tests which are negative. Sometimes the urine test indicates an infection with white blood cells. They usually give me cipro or levaquin for a week or two and it clears up. However this is the second time where I seem to have something longer term. I ve been on both cipro and then levaquin but I still have the burning sensation. I went to a urologist and he said everything was normal and prescibed more levaquin. He did a urine test, examined my prostate and a bladder scan. We tried several courses of cipro, levaquin, doxycycline, and something for trich. Last time it lasted upwards of 6 months or more and we re not sure what resolved it. I have the burning sensation again and GP did urine tests and STD tests. They were negative and urine tests did not indicate any bacteria. This time, cipro didn t seem to have an effect. Levaquin seemed to have reduced the burning sensation but hasn t gone away completely. I don t believe I have any prostate/rectum pain but then I start to second guess how I feel and wonder if I do. Although the reduction in the burning sensation could be in my head. I have been refraining from sex but do masturbate once in a while. The burning sensation is worse for a couple days after I masturbate and then seems to improve a bit. My GP is at a loss and recommended I go to the urologist again. Im fully expecting the urologist to find nothing and be in the same boat. I ve read some cases of prostatitis require longer treatments of antibiotics (upwards of a month or more) but my GP doesn t seem to think that is relevant. Do you have any suggestions? Thanks

* As per my surgical experience, the issue sounds to be most likely chronic prostatitis, which requires:

- Urine culture
- Culture of prostatic massage fluid
- Diagnostic cystoscopy
- Culture specific antibiotics, urinary alkalizer and other supportive treatment will give results over time.
