What Causes Rectal Abscess With Abdominal Pain?

My daughter's partner has recently suffered an episode of acute abdominal pain, followed by an episode that appeared to be something like a rectal abscess that burst, producing blood/pus. He has just visited his GP who examined him and could detect nothing abnormal, whilst he believes that the 'abscess' is beginning to swell again. One point of interest is that he lived in Africa for over two years ... living very much a 'native' life. He has had a number of boil-like lesions on his face and behind one ear. He is aged 30, appears thin and sallow. I've advised that he returns to the GP to see another GP, or looks for a walk-in anal/rectal clinic. Any ideas ?

General Surgeon 's  Response
Hello,

Abscess at anal verge should be taken as a case of fistula-in-ano unless proved otherwise.

Hence, I would advise him the following:

-To consult a clinic for ano-rectal diseases for clinical evaluation
-Per-rectal examination and ano-proctoscopy
-MRI fistulogram is a noninvasive procedure
-A course of an antibiotic, Metronidazole and probiotics
-Laxatives, if stools are hard
-Sitz bath in a tub with warm water
-Surgery to be planned if advised so

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant
