What Causes Periodic Pain Around The Pacemaker?

I have had a pacemaker for ten years. Periodically I have pain around the pacemaker but my dr has never seemed really concerned with it. Throughout the past few days the pain is occurring more often and the pain goes into the arm and around the pacemaker. Should I be concerned?
