Red spots but you didn't mention the site , where , these are ?
If on the butt ,it may be
staphylococcal infection as boils , frunculosis or
pyoderma (
skin infection ).
Diagnosis can be confirmed by physical examination by a
dermatologist .
When such cases visit I prescribe them to take althromycin 500 mg twice in day with acelcofenac 200 mg C R .
Both tablets two time in day for 5-7 days .
But if this red spot is just close to the anus may be
hemorrhoid or fissure in anus ( as you mentioned that there is deep burning and excruciating pain so may be due to fissure in anus or hemorrhoids ).
Diagnosis can be confirmed by physical examination by a physician .
So deal as mentioned above .

Infection on the face may be contagious or not , it depend on type of infection.
