What Causes Painful Lumps And Hair Thinning On The Scalp?

My son just showed me a patch of thinning hair in the front portion of his head. upon further investigation, I noticed a serious lump/bump where the hair is thinning. It is not hurting. He is 16. I am scared and wondering if I need to go straight to the ER. Please advise!!
