What Causes Pain In The Stomach And Back Despite Taking Pantoprazole For Hiatus Hernia?

have a sliding hiatus hernia, with frequent pain in stomach and acidity, usually gets better with Pan 40,but lately doe not respond to medicines, also taken antibiotics, now the symptoms are pain in back near mid spine, more on lying down, what could be the reason for change in location of pain.

In sliding hiatus hernia, acid is allowed to reflux back into the esophagus causing inflammation of the lining of the esophagus and the symptoms of gastro-esophageal reflux disease (GERD) such as heart burn, nausea vomiting, and burping. Other symptoms are dull chest pain, shortness of breath, and palpitations due to irritation of vagus nerve.
Symptomatic treatment is sufficient. For resistant cases, surgery is required. But there are some preventive measures you can adopt:
1.     Continue Pan 40 daily empty stomach as suggested by your doctor
2.     Syrup Mucaine gel– 2 teaspoonfuls three times daily per oral. Take this when you feel much heart burn. Otherwise avoid it
3.     Domperidone 10 mg per oral when you feel nausea
4.     Consult a gastrointestinal surgeon who will further guide you about any surgical correction of hernia if needed
5.     Avoid alcohol, smoking, spicy food, and non-steroidal pain killer
6.     Avoid eating 3 hours before lying down
7.     Do not take heavy meal at a time. Divide your meal into smaller parts
8.     Raise the head of your bed
9.     Investigation – Barium meal X - ray
