What Causes Pain In Scrotum?

I have pain in my scrotum on the right side and the pain sometimes is also present in the lower abdominal area on the right side as well and is very random. I have been dealing with this pain for over 2 years now and it effects the way i live my life a great deal. I have seen many doctors and urologists yet none of them have been able to find anything wrong(even though i feel like they didnt really care). I tried just about everything i could think to do myself to try to get rid of this pain but nothing worked. I quit exercising and working out because the pain was just too great and also had to quit my job about 6 months ago because i just couldnt handlt the pain(i worked at fedex loading trucks). However, In the past couple of weeks the pain has decreased a great amount and about 3 days before i noticed the decrease in pain i had quit smoking cigarettes. Then once the pain started to decrease i tried to start going on runs and working out again, which i could actually do this time(with little pain) compared to before when i had to stop about 15 minutes into my workout because of the intense pain. Could smoking cigarettes have been the cause of my pain and will it go away on its own now or could this pain be due to something else? (however i do still have pain occasionally but it mostly comes as soon as i wake up, or it comes when i am doing absolutly nothing but just sitting around.)

Dear I read your query and reviewed it with context to your query facts.
Based on the facts of your query, you seem to suffer from-
Varicolcele on right side.The Niacin in cigarretes creates Vasodialation effect from PGE2 and PGI2 on vessels smooth muscles.
This causes more retention and flow throught the Varicocele of the right cord and testicular pain.Waking up causes pain as you rise up causing more retention of blood by gravity,which leads to more testicular pain.Stretch receptors in the tissue cause pain in the spermatic cord and the testes.
The smoking stopping has reduced this blood flow retention caused from Niacin effect.
This is how your right testes was paining.Do Color Doppler dual image Scan with USG study of the right Spermatic Cord and the testes.This test would fix the cause of this scrotal pain.
Second opinion from Surgeon would rule out or confirm the chances of Varicocele.
