Hello and welcome to HCM,
The history suggests that the fistula has been excised and peri-uretheral abscess has been drained.
Both the lesions were infected and thus treatment involved surgical excision as well as antibiotic cover.
Pain in the anus while urinating could be due to persistence of infection.
Pain during urination could be due to existence of fistula between anus and urinary tract.
I suggest you to get an urine routine examination to look for any evidence of infection.
At the same time you need to consult your surgeon for clinical assessment of the operated site.
There is possibility of infection at the site.
Antibiotic treatment will be required to control the infection.

Thanks and take care
