Hello,

You have 2 different problems:
1) Swollen and painful abdomen
2) Hard stools with bleeding per rectum.

The following are few causes of swollen abdomen:
1) Ascites or fluid collection inside the abdomen
2) Any ovarian cyst rapidly growing
3) Enlarged liver
4) Any abdominal tumor
5) Intestinal obstruction
6) Irritable bowel syndrome

You need to consult a general surgeon and undergo thorough clinical examination and if necessary an ultrasound imaging of abdomen to point at diagnosis and initiate treatment.

Coming to your second problem, blood in stools with hard stools, The following are few causes:
1) Anal fissure (very painful condition)
2) Hemorrhoids or piles (mostly pain less)

The following are treatment options:
1) Prevention of hard stools is the key. For this, you need to use laxatives like syrup Looz 15ml once daily before sleep.
2) Eat high fiber diet.
3) If bleeding is heavy, you can take a tab. Tranexamic acid 500 mg twice daily for 2 days.

Even with all these, your problem persists, you may need to undergo surgery to remove piles. Please do consult a surgeon for further clinical evaluation.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Pallavi M., Anesthesiologist
