What Causes Low Sodium And High Creatinine Levels?

I am 74 and my last sodium level was 123. I have Type 1 LADA and on insulin pump. Last A1c was 6.8. I am on Lasix and spironolactone. My last kidney function was 36. 9 mos ago was 41. I also had chemo for breast cancer. My sodium levels have been a little for years. Do you have any idea what is going on?
