What Causes Less Urination, Jaundice And Frequent Sucking In A Newborn?

Breastfed first baby three days old in a tropical climate, only one wet nappy in 10 hours. A little jaundiced, quite sleepy. Frequent sucking. Mum has sore nipples and has supplemented 15 mls of formula over six hours. The fontanelle is not exactly depressed but not the opposite either. Should we take action?
