As pr the short history that your have provided about your Mother the following can be reasons which needs to be confirmed after proepr investigations. small perforation in one ear with headache on the same side can be due to a condition called mastoiditis or development of inflammation or an abscess in the brain or its coverings. This can be diagnosed by ENT Endoscopy and CT scan or MRI of the mastoids. Once the cause is found you can have specific treatment as per the cause found. Since your Doctor has suggested about complicated surgery means there is a possibility that CT or MRI has already been done and the report is not available here.
Take care. Hope I have answered your question. Let me know if I can assist you further.
