General & Family Physician 's  Response
Hi
The problem you present might not be a urological problem.Poor response to anti-fungal suggest a fungus isn't the cause.I will suggest you get the area examined by a Dermatologist. It sounds like scrotal dermatitis since it is an entity easily mistaken for fungi and since it responds well to zinc oxide at times. In the meantime, i will advise you put on loose dresses/panties.I will like the Dermatologist to judge the use of mild potency topical corticosteroids and antihistaminines.
Hope my answer will help you
Best regards
