What Causes Itchiness Around The Scar On The Skin Over Tibial Bone?

I have a very old scar on the skin over tibial bone medially. since 3 months, it is itchy to the extent that I have to scatch it till removing the superficial delicate skin/healel weak skin layer. Then, it heals again till the point it is itchy again to scratch. I have used topical antibiotic which accelerate healing but every time it is healed, i feel the itching again & again to scratch.

An old scar which is itching is suggestive that it has most likely turning into a keloidal scar.
Keloid is due to excessive formation of collagen as part of the wound healing process. Keloids are usually raised unlike normal scaring which is flat/level with the skin, are hard/ indurated and felt as a cord or knot on palpation.
Keloids are usually itchy.
The tendency to form keloids is inherited.
Keloids are best managed with intralesional steroids e.g intralesional triamcinolone acetonide, which is a depot injection.
The injection causes breakdown of the excessive collagen tissue present in keloidal scar, thereby causing them to flatten. The injection might be repeated after 3 weeks if required.
Other options are CO 2 Laser vaporization; 5-Florouracil (5-FU) Injection.
Symptoms such as itching and pain reduce considerably as the keloid begins to flattens.
