General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

His symptoms and lack of medication compliance are suggestive of worsening psychotic episode. Please be advised to take him to the emergency room to initiate treatment with antipsychotics. If he is noncompliant, you must also consider switching him to injectable antipsychotics in the long term.

Regards
Dr. Lekshmi
