24YO White male, 1/2 pack a day smoker x 2.5 yrs, works out, eats healthy and organic. Symptoms: Migraine x4d prior to episode of chest pain, SOB, feeling of impending doom, pain in the left wrist and elbow. Quick onset upon getting out of vehicle. Troponin 0.09 1st draw, 0.06 twice after initial draw 12hrs apart, D-Dimmer 1.34. CT of chest shows no signs of PE, EKG is normal x24 hrs straight, after and stress test the following day. MRI of brain and spine are unremarkable. 2 days after is having hot and cold flashes and profusely sweating and shaking throughout his entire body occurs mostly @0200-0400. Muscles of neck and pack are really stiff limiting ROM. Still has a intractable headache that limits amount of daily activity. has lasted12 day thus far. Any thoughts or ideas would be appreciated.
