General Surgeon 's  Response
Hello,

* As per my clinical experience, the painful hemorrhoids with vaginal swelling may be in relation with

- 3rd degree prolapsed hemorrhoids
- anal trauma through anal intercourse ( if involved )
- peri anal abscess with cellulitis
- severe vaginal or cervical inflammation

* In my clinic, I do recommend per rectal , per vaginal and anoscopy evaluation for the final diagnosis and curative guidelines accordingly.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr. Bhagyesh V. Patel
