General Surgeon 's  Response
Hi,

Noted the history about your 28 year old brother having green loose stool, and blood at the end, Raised WBC, stool showing mucus, treatment not helpful.
All this indicates that he is suffering from Enterocolitis.
The bleeding is due to infection of the intestines including rectum.
I would advice him the following in such a situation:
Send the blood for culture and sensitivity.
Change over to a proper antibiotic active against intestinal infection.
Consult a Specialist if needed.
Plenty of oral fluids.
Probiotics.
If needed get him admitted to the hospital for proper fluid, electrolyte balance and intravenous antibiotics as needed.
Ultrasound of abdomen.
Colonoscopy at later date.
Further management as per the clinical evaluation and reports of the investigations.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
