What Causes Fluid Around Lungs After Open Heart Surgery?

my dad passed yesterday, 16 days ago he survived open heart surgery which included a valve replacement quad bypass and aorta streghthening, he was released to a rehab facility and was doing well they took him to hospital for a chest x ray and blood work, he became unconsious and passed away from fluid in and aruond the lungs my question is could this have been prevented?
