What Causes Fatigue Along With Appetite Loss And Leg Pain?

In May of this year, my father who is 77, was extremely active with the stamina of a 50 year old. He started feeling unwell but would not call his doctor. He thought it would pass. He went on like that, feeling weak and tired. Finally he agreed to go to ER with me and they admitted him and put him in ICU the following day. The hospital said his sodium was critically low and he could have easily died. He was almost delirious at that point. He has type 2 diabetes and when they released him from the hospital, they now have him on a 4x per day insulin regimen. He was released on June 19,2017. The hospital told me it would take several months to regain his strength. The first month he seemed to do a bit better. This past month he has gone in the other direction. He has no energy, no appetite, his legs hurt, he has a tremor, his coloring is grayish, his blood pressure has dropped, and now his ankles and feet are swollen. I am very concerned because I live two hours away. I only get to see him on weekends. I called his doctor and insisted I bring him in last week and got to see a PA. She ran blood and said his iron was low and she lowered his Lisinopril from 40mg to 20 mg. His sodium was 133. So, no real explanation. Should I insist on a certain specialist? If so, what type? Cardiologist, Vascular? Please advise me because I don't want to feel that I should have done more. I just don't know what avenue to take.

"As"per your clinical history is concerned please follow like this -

1)Do a physical examination by his treating Doctor [Cardiologist]and do few investigations like [ECG,ECHO,TSH and CBC]

2)Do take prescribed medications as per clinical findings only.

Do take few lifestyle modifications like this-

1)Try to avoid smoking

2)Try to control certain conditions, such as high blood pressure and diabetes and take medications regularly.

3)Try to be physically active

4)Do take healthy foods

5)Try to maintain a healthy weight

6)Try to reduce and manage stress
