hi
the symptoms which you say can be mimicked either by a cardiac event,or a respiratory event or gastrointestinal event

all these symptoms can be because of gastritis ,acid peptic disorder , a duodenal ulcer has a typical history of waking up at 2 am in morning with a feeling of constriction heaviness associated with swalloinhg difficulty due to gerd
as you have fever which can lead the diagnosis towards a respiratory yract infection like pneumonia

thirdly it can be a stable angina as it also has all these symptoms
my suggestion depends on your history
your age smoking habits history of cardiac disease in family
if you have acid gas problem or gastritis

adv
ecg
cbc
tlc
dlc tests
follow up with reports and details
